I know Gunstock has one and I've tried it. They put you through a tiny two stage training course, then you ride up the high speed quad and there are three lines down the mountain. They make sure you're strapped in safely, though, and I didn't feel very unsafe riding it at all. I figured that she would have fallen off somehow, but upon reading it it said something about the waist? My guess would be strapped to tightly and possibly not being held up by something else.

Bromley has one which is massive! You ride up the "Alpine chairlift" (Aptly named, it services the zipline and alpine slide) and you go through many safety checks. It was awesome. They tend to make you do lots of checks before hand. The thing is at least 60 miles per hour. The first time I did it, I wore a hat and I was holding it down so it wouldn't blow away (It almost did in the first stretch). Here is a video (It starts at 1:00, up to that point they are getting in the harness, getting set up, and waiting for their turn to go).

It was uber safe, probably because it was 200 feet above the ground and they don't want to get sued.

Zip lines are normally safe. As long as maintenance is being done as it should and operations have trained staff.
It sounds like a tree breach is the reason for the death. Micro gust of wind. SHe was in the wrong place at the wrong time. (everything happens for a reason / someone has a plan for everyone...)
