Early reverse operation gone awry. Long before detachable lifts had signs saying 'this lift may run in reverse', Hyak would sometimes disable the backstop and use the reverse gear on the aux engine to bring misloads back to the station. In this case the operator apparently neglected to re-engage the backstop and as soon as he hit the start button and the service brake opened, the lift ran backwards uncontrolled. Only a few serious injuries, luckily enough.
